Complete a Financial Assistance Application to apply for discounted or free care based on your family size and income/assets as compared to the Federal guidelines.

SELF-PAY DISCOUNT

PCMH offers a 20% discount to all patients who

do not have health insurance. The discount is

provided only if an account is paid in full within 30 days

of receiving the first statement. This provides a

comparable discount to insured patients.
